Study aim of the objectives

The main objective of the project is to train and equip CHWs with innovative digital technology to be able to provide home and center-based care for chronic care management, supporting outbreak like COVID-19 preparedness and response in Rwanda

Specific objectives include:
*Mobilize CHWs to raise awareness, rumors management, sensitize the community to Covid-19 prevention and mitigation measures, in particular in vulnerable people (elderly, patients with comorbidities, etc) ;

*Involvement of CHWs in the detection, active case searching, mass testing mobilization—contact tracing, case investigation;

*Psychosocial Recovery Support and Reintegration of recovered patients

*Strengthen the community based surveillance for mortality that might be due to Covid or other future public health crises especially during lock-downs.
